Case Management Notes
Targeted Case Management agencies use the Case Management Note to record activities in SETWorks:
To use the Case Management Note:
First, select the Department.
Filter the Consumer list by Primary and/or Secondary Contact (Optional) - displays only the selected users Primary and Secondary contacts.
Select a Consumer.
Select the Date of Service.
Enter the duration of the appointment and any travel time (optional)
Select the Service or Service Code.
Select the Person Seen. Options are populated through utility.
Select the Place of Service.
Complete the optional checkboxes:
Designated Therapist - Denotes whether the note is billable. (Deselect to create TCM note under a billable service but is excluded from billing. Used for training purposes.)
Include on Monthly/Quarterly. (Deselect to exclude note from Monthly and Quarterly forms.)
Disclosure - used to flag case management note for follow up for legal disclosure. (Reflected on Case Management Note Report).
Unlike Activity Records, Case Management Notes do not start and end at a specific time of day. Record the duration of the appointment, not the start and end time.
Unless the case note should be excluded from billing, ensure the Designated Therapist checkbox is selected.
If Place of Service is set to Nursing Facility the note will not be able to be selected for billing.
