Creating a Question

Creating a Question in Form Builder

There are many types of questions you can create (Headers, Lists with check boxes and drop down style lists just to name a few of many), but for this section of the Form Builder tutorial we are just going to go over the basic setup of a question.

Terminology can be modified within SETWorks to better fit the needs of your organization. If the terminology in our articles are not the same on your site, talk to your local administrator to assist in translation.


Creating a Question


Select "Add new record" in the "Questions" section



This will open the "Create Form Question" window




  • Question Description: Enter the text you would like to appear as the first question in this field.
  • Question Type: The only option that will be available by default is FreeText until other Question Types are added (see: Adding Question Types)
  • Question Control: This option controls the way your question is displayed or behaves (e.g.: Bold Text, Checkbox, Single Line Textbox). See the Question Control solution page for details on each option. 
  • Precedence: This is the order that the question will be displayed on your form.
By default, the form builder puts question precedence in order by 10. This is done automatically so that if a question is left out or needs to be added there is space to do so. 
  • Default Value: Any text that is entered into this field will show up by default when the Question Control is set to Single Line Textbox or Multi-Line Textbox.
  • Active: This box is checked by default, if this box is unchecked the question will not be displayed on the form.
This is an option so that is a question no longer applies, it can be made inactive at a later date. 
  • Display on Report: This box is checked by default, if this box is unchecked the question or its entered data will not be displayed on the report.
An example of why you may not utilize a question on a report and this box would be unchecked is if you want staff to enter notes about a question or section for internal record keeping purposes but you do not want the data to be displayed on the form report for external sources to read.
  •  Answer Required: Checking this box makes answering this question required before the form can be saved or updated.

Question Dependencies

Show this question when ALL of the below conditionals are true (e.g. Gender is Female)

(Currently the only option is "consumer is 18 or older")

Choose this dependency from the drop down if you only want the question to be displayed if the client/consumer is 18 years or older. 


Show this question based on answers to other questions

This controls if you only want this question to be displayed if certain options were chosen in previous questions. (See Question Type Creation & Question Control Options to create custom dependency choices)
