Question Control List
Below are all of the Question Controls listed in order, by name and with examples of how they appear on the form builder.
Question Control - Styles
These are question controls that determine the way text will look within your form and on it's respective report but do not have any features that the user can interact with when entering data into a form:
(Question Control: Signature Line)
The Signature Line only provides a blank line for physical signatures to be added to the form's report when being printed.
Interactive Question Controls
These are question controls that give the user that is creating the form the ability to interact with questions and/or enter data:
(Question Control: Check Box):
Creating Custom Question Types with Question Options
Some Question Controls require additional setup by creating custom Question Types, in this tutorial we will be using Colors as an example.
Navigate to the Question Types section of the Form Builder and click on "Add new record"
- Description: Here you will enter the name of the list you will select from the Question Type dropdown within the Question Creation window.
- Code: This is a short version of the description in all capital letters and any spaces are replaced by underscores (e.g.: COLORS_CODE)
Now that we have setup our Question Type, it is now time to add our Question Options:
Navigate to the Question Options section of the Form Builder and click on "Add new record"
- Question Type: By default, the most recent Question Type you created will be shown here.
- Description: In this field, enter your first option from the list you are creating.
The Checkbox List & Radio Button List are the only Question Control options that can incorporate the Include Option Comment setting. (see examples at the bottom of this page)
We recommend the precedence to be numbered in "10's", that way you have 9 spaces between each option for adding or moving items around if mistakes are made (e.g.: 10, 20, 30, 40, etc)
- Active: The active box must be checked in order to be able to display this Question Option as a choice in the form.
Examples of the custom Question Type "Colors" with Question Options Red, Blue, Yellow & Other along with how their corresponding Question Control choice behaves on your form:
Question Control: Dropdown List (only allows one option to be chosen)
Question Control: Checkbox List (allows the selection of multiple options)
Question Control: Radio Button List (only allows one option to be chosen)
Below are examples with the Include Option Comment box checked.
This provides a corresponding blank textbox for further details to support the user's answer:
Question Control: Checkbox List
(allows the selection of multiple options)
Question Control: Radio Button List (still only allows the selection of one option)
