Appointment Forms - Configuring Appointment Forms

How do I configure Appointment Forms?

The Form Builder is used to create & maintain your agencies forms. Appointment Forms are just normal forms with a few additional settings configured, and therefore are configured in the Form Builder. This article will explore how to configure Appointment Forms.

Access the Form Builder Utility

  1. Click on the Settings icon in the top right of the navigation bar.
  2. Select the Provider Build tab.
  3. Select Build Forms.


Create a New Form

The creation of the form is the exact same as any other SETWorks form. For more detailed information on creating SETWorks forms please review Creating a Form in the Form Builder. Below will illustrate the specific configuration steps necessary to turn a form into an Appointment Form.

  1. Click + Add new record in the bottom left of the grid.
  2. Complete the following fields:
    1. Description -- the name of the form
    2. Entity Type -- Consumer
    3. Use Single Date Not Date Range -- this field MUST be selected
    4. Rename Date Field -- optional depending on workflow. In this example we are creating a Consumer Medical Appointment form, so we are renaming to "Appointment Date"
    5. Show Start and End time on Forms -- this field MUST be selected
    6. Appointment Form (Displays form on calendar)  -- this field MUST be selected
  3. Click save/update.


Add Form Questions

Completing the above steps is all that is necessary to create an Appointment Form. Feel free to add pertinent questions as needed for your documentation requirements.
