Service Delivery Outcome Report

The Service Delivery Outcome Report is a report required for use by the Division of Vocational Rehabilitation (DVR) in Washington. This report can be completed as a form within a Client's Profile in SETWorks. This Form matches the formatting and is accepted by Washington DVR.

How to Complete the Service Delivery Outcome Report

SDOP Dates

In order for the form to populate correctly, the Client needs to have a Client Event for their SDOP Dates. This Client Event will need the code of "SDOP".

SDOR Form Details

There are a number of fields in the SDOR that populate with information that already exists within SETWorks.
  1. AFP Number - The Authorization Code from the Authorization selected on the Form.
  2. DVR Customer - Client's Name
  3. Social Security Number - Last four digits from the Social Security Number from the Client's Profile.
  4. CRP Name - Provider name from Provider Details (Settings > Provider Build > Provider Profile).
  5. CRP Representative - Primary Assigned Staff from the Client's Profile.
  6. DVR Counselor - Funding Source Contact from the selected Authorization.
  7. Dates of this Reporting Period - From and To dates are populated from the dates selected at the top of the Form.
  8. SDOP Timeline - SDOP Dates from the Client Event on the Client's Profile.
