Opening and Completing a Form

How do I complete a form in SETWorks? 

Managers
Once forms are developed, they can be accessed depending on the type of form and the purpose it serves. See Working with Forms to learn about the different types of forms available in SETWorks.
Terminology can be modified within SETWorks to better fit the needs of your organization. If the terminology in our articles are not the same on your site, talk to your local administrator to assist in translation.

Accessing Consumer Forms

1. Select the Consumer index at the top of SETWorks. 
2. Navigate to the Consumer of choice, and select to open their profile. 
3. Select the Forms tab on the left of the Consumer profile. 





Adding a new form

Within the forms tab of the consumer profile, 
1. Select the form of choice from the Forms dropdown.
2. Select +New Form
3. Complete the form by inserting information as requested. 
4. Save.
5. Select View Report to view the completed form.










Priority Forms

Priority Forms is an quick and easy way to create forms from anywhere in SETWorks. Accessed through the global menu, you can create forms wihtout navigating to the individuals' profile.
To enable Priority Forms, the following privilege must be enabled Settings > Maintain Privileges > Forms > Enable Priority Forms
1. Click on the select-add global drop-down menu from any page
2. Select + Priority Form
3. Select the form you want to create from the form drop-down
4. Select the consumer you wish to create the form for
5. Complete form by inserting information as requested
6. Save
