Missouri Monthly (formerly: Medicaid Waiver Summary)

What is the Missouri Monthly (formerly: Medicaid Waiver Summary) form for?

The Missouri Monthly is a form used to write a summary of progress made over the last month. 

To create a Missouri Monthly form, follow these steps:

First, navigate to any consumer and select the forms tab. 
In the forms dropdown, search for Missouri Monthly (screenshot):



Select the Missouri Monthly (formerly: Medicaid Waiver Summary) and then click +New Form. 

At the top of the form, select the first and last days of the month you're reporting on. 

Select the applicable authorization(s) and then start filling out the form (screenshot):




The form will automatically display all goal comments that were entered during the reporting period so that you can review the notes and write a summary. 



Once finished, the daily goal comments will not be displayed on the completed form. Only the actual goals and the summary you write in the form will be displayed. 

When finished writing a summary of progress made on each goal, save the form by selecting save (or update) at the top of the form. 

Add a signature, if desired. 



To view the finished form report, select View Report:

Troubleshooting

Are you encountering an error message when viewing the finished report? 
One common issue to look out for is Inactive Goals. If a goal was Active when the form was created but was later made Inactive, this can cause errors on the report. Please check for any inactive goals, outcomes, or action steps and re-activate them. Remember, we only recommend making an outcome, goal, or action step inactive if it was entered by mistake or is a duplicate. As long as it was a valid goal at some point, we recommend leaving it Active and letting it expire if it is no longer needed.
