Missouri EVV - The Memo Field

What is the Memo field as it relates to EVV services such as Personal Assistance in Missouri?

Missouri (MO) Department of Mental Health (DMH) requires that all Electronic Visit Verification (EVV) visits contain a memo field. 

The memo field is automatically generated using notes from the activity record. 

What information is included? 

  1. Activity Record Group ID Number 
  2. Consumer Comment 
  3. Goal Text (Goal Short Description) 
  4. Goal Comment 
Per Sandata limitations, only the first 1024 characters will be included.  

What will this look like? 

Sandata might receive something like this: 
"Memo": "SETWorks Record ID: 1234567890. Comment: Kevin asked to visit the store so he could purchase a soda today. Goal: Kevin will interact with his local community 1 time per week. Goal Comment: Staff assisted Kevin with the walk to the store and provided verbal prompts to direct Kevin to the store. Kevin made his purchase and interacted with the cashier.”
