Form Result Report - Basics

How do I see results of my completed Forms? 

Administrators, Managers
Upon completing a form, there are reports in SETWorks that will aggregate your results. These reports are known as Form Result Reports.

The form result reports perform differently depending on the selected form. Sometimes the report may take a long time to aggregate results. Sometimes the form result may not be available at all for the selected form. 
Terminology can be modified within SETWorks to better fit the needs of your organization. If the terminology in our articles are not the same on your site, talk to your local administrator to assist in translation.

Access Form Results Reports - Reports Utility

- Select the Settings gear in the top right of SETWorks.
- Select Provider Build.
- Select Configure Report Access.
- Using the search bar, type Form Result to return results.
- Select the report of choice depending on the type of form used.
     (e.g., Consumer Form Result Report shows form results for only consumer forms)



Use the Form Results Report

- Select the Form Results Report to open.
- Select the From. date. 
- Select a To date.
- Select a Form.
- Select Staff. Optional
- Select a Consumer Status. Optional
- Select a Consumer. Optional
- Select Generate Report
