How to Populate the Emergency Fact Sheet Report

All Users

The purpose of this document is to show from where in the consumer profile each field in the emergency fact sheet report (EFS) is populated. A large portion of the data compiled in this report may be entered into the consumer's profile upon creation of the consumer. This information may be modified at any time by clicking on the edit record link in the consumer details tab of the consumer's profile. 

Please Note: Clients with customized emergency fact sheet reports, the directions outlined in this article may not apply to every field in a customized version of this report.

The following fields are populated from the Consumer Profile intake/edit screen within the Primary Information tab: 
Name
Nickname
DOB
Medicaid ID
Sex/Gender
Race/Ethnicity
Language/Communication
Transportation
Primary Disability

From the Contact Information tab: 
Current Address

From the Additional Detail tab: 
Medicare 

Age is calculated from the DOB field. 



From the Housing Education/Legal tab 
Legal Competency Status displays "Competent" when the "Is own guardian" field is checked.



The following fields are populated from the Characteristics tab in the Consumer profile. 
Height
Weight
Build
Hair
Eyes
Distinguishing Marks
Ability to Protect Self Without Assistance
Significant Behavior Characteristics
Likely Response to Search Efforts
Pattern of Movement (if Lost Previously)
Places Frequented
Relevant Capabilities
Limitations
Preferences




The following fields are populated from the Network/Contacts tab in the Consumer profile: 
If Legal Guardian, Name/Home Phone/Address/Cell/Work information is populated where the contact type code must be GUARDIAN.
This contact type code may be updated by administrators in the Contact Type Utility .




Primary Emergency Contact/Family/Guardian/Residence
Secondary Emergency Contact/Family/Guardian/Residence  

The address/phone of the two Network Contacts that have the "Emergency Contact" checkbox checked (found in the Additional Details near the bottom of the modify contact window) will appear in these fields.
NOTE: If only one Emergency Contact is added, the Secondary Emergency Contact information will remain blank.
NOTE: If more than two Emergency Contacts are added, the system will choose which ones to populate at random.

The following are populated from the Network/Contacts > Doctors tab in the Consumer profile:

The assigned doctor type code must be PRIMARY_CARE.
NOTE: Edit the Doctor Type code within the Doctor Type utility







The following fields are meant to be completed upon printing the report: 
Probable Dress  
Where and When was the person last seen?/Date/Time
