AbilityOne Functionality in SETWorks - How to

Administrators, Managers
SETWorks can now populate two required AbilityOne forms: Documentation of Disability and the Individual Eligibility Evaluation form (see below). In order to populate these forms, the first step is to contact the SETWorks Support team to enable the AbilityOne Approver Role. The following steps are listed within this article. 

Terminology can be modified within SETWorks to better fit the needs of your organization. If the terminology in our articles are not the same on your site, talk to your local administrator to assist in translation. 

What is the AbilityOne Program?
The AbilityOne Program is among the nation's largest sources of employment for people who are blind or have significant disabilities. The Program is administered by the U.S. AbilityOne Commission, the operating name for the Committee for Purchase From People Who Are Blind or Severely Disabled. Through a national network of approximately 500 nonprofit agencies, the program provides the Federal Government products and services at fair market prices. The procurement of these products and services results in employment of approximately 45,000 individuals

NOTE: You need the AbilityOne Approver Role in SETWorks to perform this task. Contact support@setworks.com to enable this role.

Creating Documentation of Disability

  1. Go to a Consumer Profile by clicking on the “Consumer” link in the top left. Select the Consumer Profile you are looking to add the Form to. 


  1. Select the “Forms” tab on the bottom left-hand side of the Consumer Profile. 

       

  1. Click on the drop-down menu and search for Documentation of Disability.


  1.  After selecting the Documentation of Disability Form click on the “+ New Form” button.


Uploading the Documentation of Disability

  1. On the table in the top right corner click the  icon to upload your Document.


  1. From the “add file” window click on the “Select” button.


  1. Find where you have saved your Document.  Click on the Document then click on “Open” button.


  2. Click on “Save” on the top right-hand side of the “add file” window.


After completing these steps, you will now see the Document is shown in the table on the top right-hand side.


Completing the Documentation of Disability

  1. The date of the document is shown on top left of the form.


  1. Document Source, Doctor’s Name/Certifier’s Name, Disabilities, and Disability as Written need to be completed. This is the information that will be pulled into the Individual Eligibility Evaluation.



"Diagnosis Approval Determination" drop down will be set by your AbilityOne Approver
"For Ability One" section is only to be filled out by your AbilityOne Approver

  1. Click “Save” on the top right of the Form.


Notify your AbilityOne Approver that they can now make an approval determination on your Documentation of Disability.

Creating Individual Eligibility Evaluation

  1. Go to a Consumer Profile by clicking on the “Consumer” link in the top left. Select the Consumer Profile you are looking to add the Form to. 
                 

  1. Select the “Forms” tab on the bottom left-hand side of the Consumer Profile. 



  1. Click on the drop-down menu and search for Individual Eligibility Evaluation.


  1. After selecting the Individual Eligibility Evaluation Form click on the “+ New Form” button.


Writing the Individual Eligibility Evaluation

Complete the following fields:
  1. Information considered pertinent to or supporting the evaluation: complete as needed
  2. Synopsis of Functional Limitations: For each diagnosis, check off each functional limitation that applies. Continue for each diagnosis.
  3. Competitive Employability: the default response is “No”. Change as needed using the drop down menu.
  4. Functional Limitation Details: For each functional limitation area checked in the section above, write details about the limitations.
  5. Supports and Accommodations: for each functional limitation detail, write the corresponding support and accommodation
All six functional limitation areas will show regardless of how many boxes were checked in the section above.

Viewing the Individual Eligibility Evaluation

  1. Go to the Consumer Forms and find the Individual Eligibility Evaluation you want to view.


  2. Click View Report.
