Utilizing Forms for Member Appointments

I would like to know how other agencies are keeping track of doctor, dentist, psych, etc. appointments through SETWorks. We have a form on SETWorks that is filled out by the staff person after the appointment. They currently take our old paper form TO the appointment and the doctor fills it out there, then we are uploading that scanned document to the form we input on SETWorks. I'm looking for a more streamlined process. Does anyone take electronic devices to these appointments and have doctors fill the forms out directly on SETWorks? Interested to hear what others are doing!
