Using SETWorks Forms for Consumer Annual Paperwork
Hello! I'm wondering about other providers' processes for creating and signing consumer paperwork through SETWorks? We have several forms that need to be reviewed, filled out, and signed each year at annual meetings. We have been doing these on paper but want to move to SETWorks. We initially created each form separately in SETWorks, but this is VERY cumbersome when sending out to parents/guardians or consumers via email. We are concerned about making a large form with all of the needed documents within that on Form, so that it only has to be sent once and signed once. Does anyone have one or two BIG forms with all of the needed paperwork items included? If so, do you have the parent/guardian add their initials or something under each separate section, or just one final signature? Just curious how others are doing this or if you keep everything separate and just send multiple forms out each year.
